Depending on your location, you have access to up to four plans: the Anthem PPO, Anthem HSA, Anthem HDHP, or the Kaiser HMO (if you reside in California).
The Anthem Preferred Provider Organization (PPO) plan covers many common health care costs with predictable copays.
The Anthem HSA Plan lets you enroll in a Health Savings Account (HSA) to save pre-tax dollars for healthcare expenses. You must meet the deductible before your plan provides coverage.
The Anthem HDHP offers lower premiums than the HSA Plan. You are not eligible to enroll in an HSA when enrolling in this HDHP. You must meet the deductible before your plan provides coverage.
If you enroll in the HDHP $5,000 plan, you are automatically enrolled in $5,000 of employee only Critical Illness insurance.
This plan is only available to employees living in California. The Kaiser HMO plan only covers visits to in-network providers. You will select a primary care physician (PCP) who will oversee your care and refer you to specialists, if needed. You are responsible for copays at the time of service. There is no out-of-network coverage with this plan. Find an in-network provider at kp.org.
Both the Anthem and Kaiser medical plans provide options for virtual care. Anthem plans utilize LiveHealth Online to connect you to a provider, while Kaiser plans use Kaiser’s internal telehealth services.
TheKey offers two PPO dental plan options through Cigna: the Low Plan and the High Plan.
The Cigna PPO Low Dental Plan provides coverage for dental expenses up to the maximum of $1,000 per year.
The Cigna PPO High Dental Plan covers up to a maximum of $2,000 each year and also provides $1,250 of orthodontia coverage.
The VSP PPO Vision Plan provides coverage for vision-related expenses and exams. You may visit in-network or out-of-network providers.
